Silent Hill: Townfall Haldane Hotel Door Puzzle Solution

After you reach the Town Square phone booth in St. Amelia's Shorefront area, call 100, and you'll be transported to a hotel room at The Haldane Hotel. To open the locked bathroom door, you need to solve a puzzle on said door that requires you to enter three things: a number, a month, and a year.
