The survey can be filled out until June 14, and the optional prize draw is entered by submitting your email address at the end of the survey. You must be a resident of the US or UK and be at least 18 years old to be eligible to win (with some exceptions listed on the survey page). More terms and conditions here.
If you're not eligible for the prize draw, we still want to know what you think, and you're welcome to fill out the survey.
